Nurse/Clinician or Clinician-in-Training

Congratulations! You’re taking an important step in your health care professional development. You can begin the application process if your institution already has a clinical affiliation with Jupiter Medical Center. If so, please fill out the training request form and ask your clinical coordinator to complete it and forward to it to Jupiter Medical Center’s Training Department. Notify the Clinical Education Department at least 3 weeks before clinicals start.

Once your training is approved, you’ll fill out additional forms:

• Student Onboarding Checklist
• Student Health and Background Screening Attestation
• Background Screening Consent Form
• Health and Background Screening Attestation for Medical Students
• Medical Exemption From COVID-19 Vaccination Form
  Religious Exemption From COVID-19 Vaccination Form
