Credentialing

To begin the application process, please send the following information, with a copy of your current CV, to MedStaffSvcs@jupitermed.com:

• Full Name
• Degree
• Specialty
• Applicant’s Email Address
• Practice Name
• Primary Office Address, Phone & Fax
• Name of Sponsoring Physician (Advanced Practice Providers only)

When this information is reviewed, you will receive a link to the online application form. Please note that we have a two-step application process for physicians. A pre-application must be completed/approved before a full credentialing application will be released.
